Interim Public Health Administrator accept funds from the State of Wisconsin Division of Public Health for the Immunization Program and the Mayor and City Clerk be authorized to sign the agreement
Resolved, that the Public Health Administrator accept funding in the amount of $33,324 from the State of Wisconsin represented by its Division of Public Health of the Department of Health Services Immunization Program, and the Mayor and City Clerk be authorized and directed to enter into the agreement with the State of Wisconsin represented by its Division of Public Health of the Department of Health Services Immunization Program (Grant Control #2009-068). The period of the grant is from January 1, 2010 through December 31, 2011.
