Title: Conditional Use Permit be granted to operate a Childcare Center at 1442 N. Memorial Drive Resolved, that a Conditional Use Permit be granted to Kimberly L. Horton to operate a Childcare Center at 1442 N. Memorial Drive, subject to the following conditions: a. That the plans presented to the Plan Commission on April 29, 2009 be approved subject to the conditions contained herein. b. That building and occupancy permits and all other applicable permits for the daycare center be requested from the Building Department. c. That signage be confined to the existing wall sign facing Kewaunee Street frontage and the multi-tenant monument sign on the N. memorial Drive frontage. d. That the maximum hours that the day care facility be open to the general public be from 6:00 a.m. to 12:00 a.m. Monday through Saturday with no hours on Sunday. e. Conditional Use Permit be granted to operate a Childcare Center at 1442 N. Memorial Drive

Resolved, that a Conditional Use Permit be granted to Kimberly L. Horton to operate a Childcare Center at 1442 N. Memorial Drive, subject to the following conditions:

a. That the plans presented to the Plan Commission on April 29, 2009 be approved subject to the conditions contained herein.

b. That building and occupancy permits and all other applicable permits for the daycare center be requested from the Building Department.

c. That signage be confined to the existing wall sign facing Kewaunee Street frontage and the multi-tenant monument sign on the N. memorial Drive frontage.

d. That the maximum hours that the day care facility be open to the general public be from 6:00 a.m. to 12:00 a.m. Monday through Saturday with no hours on Sunday.

e. That all yard areas, landscaping, buildings and grounds be maintained in a professional manner and on a daily basis.

f. That all employee parking be contained on site.

g. That all children shall be unloaded from vehicles, and loaded into vehicles, in either the parking lot or on the south side of Kewaunee Street.

h. That all trash and recyclables be stored in stored closed containers and screened from view.

i. That all buildings and grounds be maintain in a professional manner and on a daily basis.

j. That all codes and ordinances be complied with and required permits acquired, including a State of Wisconsin daycare license.

k. That no minor changes be made from the conditions of this permit without the approval of the Plan Commission and no major changes be made from the conditions of this permit without the approval of the Common Council.

l. That this Conditional Use Permit is subject to review by the Plan Commission for Compliance with the listed condition.
