Subject: Communication from the Public Health Administrator requesting permission for the Mayor and City Clerk to sign a contract with the State of Wisconsin Department of Health and Family Services, that will provide funding for West Nile Virus surveillance activities. The contract is for $2,000 and there is no match required by the City.
Recommendation of the Board of Health 06-13-06: That the Mayor and City Clerk be authorized and directed to enter into an agreement with the State of Wisconsin DHFS, Division of Public Health in the amount of $2,000. The funds will be used for West Nile Virus surveillance activities from 1/1/06-12/31/06.
Fiscal Note: There is no match required on the part of the city.
Recommendation of the Finance & Personnel Committee 05-08-06: That the Mayor and City Clerk be authorized and directed to enter into an agreement with the State of Wisconsin Department of Health and Family Services, Division of Public Health (Grant Control No. 2006-021) in the amount of $2,000 to be utilized for West Nile Virus surveillance activities to run from January 1, 2006 through December 31, 2006.
