Conditional Use Permit for 304 Main Street
Resolved, that a Conditional Use Permit to add massage therapy services at 304 Main Street be approved, subject to the following conditions:
a. That the plans presented to the Plan Commission on May 13, 2009 be approved subject to the conditions contained herein.
b. That all applicable building and occupancy permits be obtained from the Building Inspection Department.
c. That the maximum hours the operation is to be open to the public are Monday through Friday, 9:00 a.m. to 9:00 p.m., and from 8:00 a.m. until 6 p.m. Saturdays and Sundays.
d. That all codes and ordinances be complied with and required permits acquired, that State approvals for massage establishments be obtained if applicable, and that only licensed massage therapists provide such services.
e. That no minor changes be made from the conditions of this permit without the approval of the Plan Commission and no major changes be made from the conditions of this permit without the approval of the Common Council.
f. That this permit is subject to Plan Commission review for compliance with the listed conditions.
