RESOLVED, that the Mayor and City Clerk be authorized and directed to enter into agreements with the Villages of Elmwood Park and Wind Point for Level 1 Health Services for the period of three years 2006 - 2008.
Fiscal Note
Fiscal Note: The cost to Elmwood Park will be $811 for 2006, $1,277 for 2007, and $1,341 for 2008. The cost to the Village of Wind Point will be $2,114 for 2006, $2,220 for 2007, and $2,331 for 2008. There is a 5% increase for each of the three years.
