Title: Request from Stephanie Kober seeking an amendment to a conditional use permit for HALO. Resolved, that the request of HALO, the Homeless Assistance Leadership Organization, Inc., for a Conditional Use amendment to allow its childcare program at 2000 DeKoven Avenue to offer services to the general public, be approved subject to the following conditions: A. That the plans presented to the Plan Commission on August 31, 2011 be approved subject to the conditions contained herein. B. That all codes and ordinances be complied with and that all required permits be obtained. C. That the number of children enrolled in the childcare center be provided to city Staff for their records. This number shall not exceed 22 at any one time. D. That there be adequate staff, as dictated by State licensing requirements, present at all times for childcare services. E. That all staff members providing childcare services be properly trained and licensed as required by the State of Wisconsin. F. ...

Request from Stephanie Kober seeking an amendment to a conditional use permit for HALO.  
 
Resolved, that the request of HALO, the Homeless Assistance Leadership Organization, Inc., for a Conditional Use amendment to allow its childcare program at 2000 DeKoven Avenue to offer services to the general public, be approved subject to the following conditions:
 
A.  That the plans presented to the Plan Commission on August 31, 2011 be approved subject to the conditions contained herein.
 
B.  That all codes and ordinances be complied with and that all required permits be obtained.
 
C.  That the number of children enrolled in the childcare center be provided to city Staff for their records.  This number shall not exceed 22 at any one time.
 
D.  That there be adequate staff, as dictated by State licensing requirements, present at all times for childcare services.
 
E.  That all staff members providing childcare services be properly trained and licensed as required by the State of Wisconsin.
 
F.  That all grounds, including the outdoor play area, be maintained on a daily basis.
 
G.  That the hours of operation for the childcare services may be 24 hours a day, 7 days a week, 365 days a year.
 
H.  That the loading and unloading of children shall not block any driveways or parking areas within the site, including access to any of the other buildings.
 
I.    That any proposed signage shall be submitted for review and approval and any required permits pulled prior to installation.
 
J.  That no minor changes be made from the conditions of this permit without the approval of the Plan Commission, and no major changes be made from the conditions of this permit without the approval of the Common Council.
 
K.  That this permit is subject to review by the Plan Commission for compliance with the listed conditions.
