Title: Canine Rescue and Adoption Center at 1535 Layard Avenue Resolved, that the request by Jodie Hoffman, representing Woof Gang Rescue, Inc. seeking a conditional use permit to operate a canine rescue and adoption center at 1535 Layard Avenue is approved, subject to the following conditions: a. That the plans presented to the Plan Commission of November 9, 2016 be approved, subject to the conditions contained herein. b. That the hours of operation for the general public be from 7:00 a.m. to 8:00 p.m. daily. c. That prior to the issuance of an occupancy permit, a landscape plan and parking lot repair, sealing, and striping plan for 1535 Layard Avenue be submitted to the Director of City Development for review and approval. d. That by May 1, 2017 the following shall be accomplished: 1. Implementation of the approved landscape plan. 2. Repair, seal and striping of the parking area at 1535 Layard Ave. e. Canine Rescue and Adoption Center at 1535 Layard Avenue

 

Resolved, that the request by Jodie Hoffman, representing Woof Gang Rescue, Inc. seeking a conditional use permit to operate a canine rescue and adoption center at 1535 Layard Avenue is approved, subject to the following conditions:

 

a. That the plans presented to the Plan Commission of November 9, 2016 be approved, subject to the conditions contained herein.

 

b. That the hours of operation for the general public be from 7:00 a.m. to 8:00 p.m. daily.

 

c. That prior to the issuance of an occupancy permit, a landscape plan and parking lot repair, sealing, and striping plan for 1535 Layard Avenue be submitted to the Director of City Development for review and approval.

 

d. That by May 1, 2017 the following shall be accomplished:

1. Implementation of the approved landscape plan.

2. Repair, seal and striping of the parking area at 1535 Layard Ave.

 

e. That any desired signage plan be submitted to the Director of City Development for review of and approval prior to the installation.

 

f. That all applicable permits be applied for and obtained.

 

g. That no minor changes be made to the conditions of this permit without the approval of the Plan Commission and that no major changes be made without the approval of the Common Council.

 

h. That this permit is subject to review by the Plan Commission for compliance with the listed conditions.
